HIPAA Business Associate Agreement Template – $49.50 – Our firm has prepared a template HIPAA business associate agreement to assist HIPAA covered health care providers prepare business associate agreements that comply with HIPAA requirements, including the new breach notification rule. Although written for providers, it can be used by business associates as well to structure their own agreements. Two documents are included. The first is a copy of the template with contains references to sections of the HIPAA regulations involved and our comments concerning the various provisions. The second document is the template agreement in Word without the references and comments so you can easily adapt it to your situation. As with any template, it is not a substitute for legal advice and legal advice applicable to your own specific circumstances should always be sought.
